If you have never seen Dr. Peine or Dr. House before and have questions not answered by this website, feel free to call the office directly at (208) 947-0925. You may also email Dr. Peine at drpeine@drpeine.com, or Dr. House at drhouse@drpeine.com.

Before your first visit, please download and complete the new patient forms located below. Please read the instructions carefully. Once you have completed the intake forms please mail, fax or drop them off at the office prior to your initial appointment. This will allow the doctors to thoroughly review your information prior to your visit. Fax number: 208/947-0926.

New Patient Paperwork

For Dr. Peine’s Patients:

Once you have downloaded and printed the forms, you may schedule a “Dr. Peine - New Patient Appointment” online by visiting the Appointments page of this website. Please read the instructions carefully.

NOTE: Because Dr. Peine has become so busy, he recommends scheduling two additional “returning patient” visits at weekly intervals after your initial “new patient” visit for a total of three appointments. This way, he can ensure that you receive timely and appropriate follow-up care.

For Dr. House’s Patients:

Once you have downloaded and printed the forms, you may schedule a “Dr. House - New Patient Appointment” online by visiting the Appointments page of this website. Please read the instructions carefully.

NOTE: Because Dr. House has become so busy, she recommends scheduling one additional “returning patient” visit 2 weeks after your initial “new patient” visit. This way, she can ensure that you receive timely and appropriate follow-up care.
